Landline Phone Service Request

I request that telephone service be added to the Dorm Room listed above, for the semester noted above. I understand that Alabama A&M University will only provide local dial tone to the room requested. The telephone device must be provided by the student. I understand that access to long distance is restricted, only collect long distance calls are allowed. I also understand that AAMU is providing this service for the indicated term at no additional charge, but that repairs to the line that are a result of negligence or abuse will be billed to my student account at the rate of $65.00 per hour. I further agree that this phone number belongs to AAMU and I may not allow any company to add chargeable services to the line (including but not limited to voice mail, or internet service). I further agree that any charges that appear on the University phone bill for the number listed below during the term indicated above will be added to my student account.

By submission of this request the above named student acknowledges agreement to all terms and conditions for use of AAMU provided telephone services.

This form will be emailed to the requestor with the assigned telephone number when service is activated.

Please allow 72 hours for phone service to be installed.
